I'm torn between the 2004 movie and the stage show. They're both awesome!
The Charles Dance miniseries is a close second.

For me, the Leroux novel and the Lon Chaney silent version.

Leroux was an author of detective fiction and probably that's why I'm intresterd in PHANTOM.
To be honest, I don' really like the romantic versions ( and yes, I know I'm in minority here :) )

Gaston Leroux's novel

Lon Chaney, Sr.'s Silent masterpiece ~ simply breathtaking, with more color than anyone could expect

2004 musical film ~ I've never seen the stage version except for the anniversary performance (My best friend, who lurks around TPO (Hi, Beth!), bought the DVD for me!)

HONORABLE MENTION: Phantom of the Paradise (1974), Brian De Palma's homage to Leroux's classic tale

Pssst! Beth! Pet Heather and Angus for me=}

Ouch! Difficult choice...
Mmm... I would say POTO 2004 movie, live show in London, Susan Kay's POTO, Gaston Leroux's novel... and so other many more that it will be too long to write them all...
